Transitional cell carcinoma (TCC) — also called urothelial carcinoma — is the most common cancer of the canine urinary tract. It usually develops in the bladder, often in the worst possible spot: the trigone, where the ureters and urethra connect, which makes complete surgical removal difficult. The upside is that TCC is unusually well-characterized at the molecular level, giving veterinary oncology both a reliable diagnostic test and a growing list of targeted trial options. What Is Canine Transitional Cell Carcinoma?

TCC arises from the urothelium, the lining of the urinary tract. Because it tends to grow in a spot that's hard to operate on, and often produces urinary signs mistaken for a simple infection, it's frequently diagnosed later than owners would like. Persistent straining, blood in the urine, or recurrent "UTIs" that never fully resolve are the classic problems worth taking seriously.

The Forms of Transitional Cell Carcinoma

Bladder transitional cell carcinoma is the most common location, often at the trigone. Urethral and prostatic transitional cell carcinoma involve the urethra or, in males, the prostate. Trigonal transitional cell carcinoma sits specifically at the neck of the bladder, the hardest region to resect. BRAF V595E–positive urothelial carcinoma is a category of its own: roughly 85% of canine TCCs carry a specific BRAF mutation, which can be detected from a free-catch urine sample with a simple molecular test. The mutation also serves as a target for therapy.

Standard Treatment and What It Costs

NSAID therapy (piroxicam, deracoxib) has genuine anti-tumor activity in TCC and forms a backbone of treatment. It is relatively inexpensive, and can be given long-term. Chemotherapy (vinblastine, mitoxantrone, chlorambucil) is used alone or with NSAIDs to shrink and control disease at $3,000 to $6,000. Procedures for obstruction, such as stenting or laser ablation to keep urine flowing when tumors block the outflow, are variable in cost. Imaging and monitoring — ultrasound and, in some cases, cystoscopy, plus ongoing rechecks — are part of long-term care. Surgery is rarely curative because of tumor location, so medical management is the norm. With NSAID-plus-chemotherapy protocols, median survival commonly falls in the range of roughly 6 months to a year, and some dogs do considerably better — but progression is expected, which is what makes the targeted therapies under study so compelling.

Clinical Trials: What Is Being Studied

BRAF-targeted therapy is a leading area of investigation, because most canine TCCs are driven by the same BRAF mutation — a rare case of a truly "matched" target in canine cancer. Folate-targeted chemotherapy exploits receptors preferentially expressed on TCC cells to deliver drugs more selectively. Novel targeted and immunotherapy agents aim to improve on NSAID-plus-chemo. And intravesical therapy delivers treatment directly into the bladder to concentrate the drug where the tumor lives.

Who Qualifies for a TCC Trial?

Most trials require a confirmed diagnosis (often including the BRAF result), imaging to define tumor extent, and adequate organ function — kidney function is especially important given the tumor's location. Some trials specifically enroll BRAF-positive dogs. Both newly diagnosed dogs and those whose disease has progressed on standard therapy are commonly eligible.
